All medical/dental appointments should be made in Sixth Form students non-contact times whenever possible, however if they cannot be, please provide a screenshot/copy of appointment letter for our records.

Please let us know the reason for the absence with as much information as possible. If reporting an upcoming appointment please inform us of the date, time and reason for the appointment and where possible provide a copy of the appointment letter.
